The concern about penis size by men dates back millions of years, and some writings even mention that male primates also have concerns about penis size. As urologists, we observe that about 85-90% of men who come to us with concerns about penis size actually have normal-sized penises. Many of these men either suffer from body dysmorphic disorder or desire a slightly longer and thicker penis due to different anxieties, even though their penis size is normal. The average erect penis size in a normal adult male has been found to be around 13 cm in many field surveys, and anything below 12 cm is considered a short penis. Non-Surgical Penis Enlargement Techniques
Various techniques are used to enlarge the penis, which we categorize as non-surgical and surgical techniques. Among the non-surgical techniques:

2- Vacuum and traction devices: There is no scientific evidence that these devices have a significant effect on permanently increasing penis size, and their use can lead to unwanted serious side effects. Surgical Penis Enlargement Techniques
Surgical penis enlargement techniques can be divided into two main categories:
1 - Techniques that alter the perception of length
- Liposuction: This procedure is performed in obese patients where excess fat in the pubic area can bury the penis, making it appear smaller. - V-Y Plasty: In this procedure, a 60-degree inverted V incision is made where the penis meets the pubic area, and this incision is sutured in a Y shape to create a perception of a longer penis from the person's own perspective. This procedure is often combined with cutting the penile suspensory ligament, which is the most commonly used technique for penis lengthening.
- Z Plasty: In this technique, a Z-shaped incision is made on the scrotum, resulting in a 70-75% increase in the visible appearance of the penis shaft.
- Ventral Phalloplasty: In this method, the penoscrotal junction is excised, revealing the part of the penis shaft that is hidden by the scrotum.
- Suspensory Ligament Cutting Technique: This is the most commonly used technique for penis lengthening, often performed in conjunction with V-Y plasty. In this technique, a reverse V incision is made on the penoscrotal area to reach the suspensory ligament that attaches the penis to the pubic bone. The entire ligament or about 85% of it is cut to advance the part of the penis shaft that remains behind the pubic bone, resulting in an increase in the visible length of the penis in both erect and flaccid states. When done carefully, the complication rate is low, making it a safe procedure. However, cutting the ligament completely may cause a loss of stability during sexual intercourse, leading to difficulties in penetration, which is the most significant side effect of this technique. Some urologists suggest cutting only about 85% of the ligament to preserve stability while still achieving a significant increase in length. This technique provides an increase of approximately 2-4 cm in penis length.

2 - TECHNIQUES TO INCREASE PENIS SIZE
- Vascular graft (penile disassembly): In this technique, the glans penis is completely separated from the corpora cavernosa and autologous vascular tissue is implanted into the resulting space. Vacuum devices are used to prevent neurovascular bundle constriction and penile curvature. On average, a 3 cm increase in penis length can be achieved. However, due to its high complication rate, this technique is not widely used by urologists and is not recommended today.
- Sliding technique: In this technique, the penis is completely freed through a combined incision, involving both penoscrotal and subcoronal incisions. The urethra is completely separated, and neurovascular bundles are secured. Bilateral longitudinal incisions are made on T. Albuginea, and then these incisions are connected with transverse semi-circular incisions. This technique is mainly used for Peyronie's disease, and it can provide an average functional penis length increase of about 3 cm. It is a complex technique, so less invasive techniques should be chosen for patients with micropenis and no other underlying conditions.
- Total phalloplasty: This technique is primarily used in severe cases of micropenis where other techniques are insufficient. It is performed in two stages. In the first stage, a free radial artery forearm flap is used to reconstruct the urethra and penoplasty to create a functional and sensate penis and urethra. In the second stage, a penile prosthesis is inserted to provide an erection.
